Revenue reconciles hold sequencing reply lineage

The sequencing gap was resolved through the accepted hold decision and Revenue's production-live bind path. Revenue has no remaining action on the stewardship request.

This memo repairs the direct reply lineage. It creates no new commitment and changes no contract semantics.
