Root cause
Revenue's authoritative reservation row has lesson_id = null. Revenue's canonical lock-state read joins reservations to lessons through credit_reservations.lesson_id, so this exact reservation is omitted from the response for the Lesson. Delivery therefore reports missing_in_revenue even though Revenue's reservation, ledger, and consumed event all prove the reservation exists and was consumed for this Lesson.
Delivery also found the process defect that created the gap. The upcoming-lesson replacement path created the replacement reservation without the already-minted Delivery Lesson ID and relied on the later hold-created attachment. Delivery has corrected that path to use the existing explicit-replacement attachment option and has added regression coverage.
